MEDICAL CODING
Medical coding is the process of converting healthcare diagnoses, practices, supplies, and equipment into standard medical alphanumeric codes. The diagnoses and procedure codes are obtained from the medical history documentation, such as the transcription of the doctor's notes, the results of the laboratory and radiologic tests, etc.
According to AHIMA, accurate and thorough clinical recording is essential for medical billing and coding during the patient contact. "Do not code it or bill for it if it is not documented in the medical record," is the cardinal rule of billing and coding in the healthcare industry. When a claim is in contention, providers use clinical documentation to support reimbursements to payers. A provider could face a claim denial and possibly a fine if they fail to adequately document a service in the medical record.
