The process of obtaining information about a patient's medical history, including treatments, diagnoses, necessary medical equipment, and services, from a doctor's notes is known as medical coding. A medical coder transforms this data into common alphanumeric medical codes.

All of the information is translated into well-defined code sets that are handled by certain healthcare organizations, including the patient name, doctor's name, hospital name, treatment, diagnosis, and medication information.
